The Saskatchewan Health Authority is working to better understand the health needs of people living in Southeast Saskatchewan. To support this effort, we have created a Population Health Survey for healthcare workers, community organizations, and individuals with experience using the healthcare system. It includes questions about local health challenges, immunization experiences, and managing chronic conditions. The information we collect will help guide future healthcare planning and improve services in your community.

We are asking for your help by completing the ‘Community Organization’ survey, using the link provided, and to share the attached poster with individuals in the community that have experience with the healthcare system, immunizations, or chronic conditions. The poster includes a QR code and link for the ‘Individuals with Lived Experience’ survey and can be printed and displayed on bulletin boards or shared with clients and community members.

The survey opens from April 22 to May 23, 2025, and takes about 20 minutes to complete. All responses are confidential and are used solely for healthcare planning purposes. Translation services or paper copies are available upon request.
